Paw`ina Volunteer Agreement

This Volunteer agreement details your agreement to be a volunteer with Paw`ina Public Awareness and Fundraising Initiative by Guide Dogs of Hawai`i (GDH), a non-profit, tax-exempt organization, as defined in Section 501 (c)(3) of the Internal Revenue Code, on Saturday, July 12, 2025, at Frank F. Fasi Civic Center. You acknowledge the following: You have the autonomy to select the volunteer task(s) you can provide and consider other GDH needs. This empowers you to contribute in a way that aligns with your skills and interests, making your contribution valuable and appreciated. I am volunteering for the organization without any expectation or intention of receiving wages, compensation, or benefits for my volunteer services from the Paw`ina Public Awareness and Fundraising Initiative. I am donating my time and effort for the benefit of the Paw`ina Public Awareness and Fundraising Initiative because I am interested in supporting the organization and its mission. My services are offered freely and without any form of pressure or coercion, direct or implied, from any Board member or employee of the organization, respecting my autonomy and decision to volunteer. You are not an employee of Guide Dogs of Hawai`i, and there is no expectation of an employment relationship, whether express or implied. This ensures that you are clear about your status as a volunteer. I understand that I will not be reimbursed for any personal expenses incurred, such as parking or meals, in performing my volunteer work. Your volunteer service is not for a fixed period, and Guide Dogs of Hawai`i may release you as a volunteer without prior notice and for any reason. This is to ensure the safety and well-being of all volunteers, making your participation secure and protected. I attest that Guide Dogs of Hawai`i does not provide food, shelter, clothing, necessities of life, or any other similar benefit. I understand that I may be required to complete a Background Investigation Disclosure and Authorization Notice and a Criminal History Disclosure Form. These checks are conducted for the safety and security of all participants, including volunteers. My volunteer service may be contingent on the satisfactory results of such checks to Guide Dogs of Hawai`i or venues where the Paw`ina Public Awareness and Fundraising Initiative is held. I understand that I cannot accept or participate in promotional games, prizes, and auctions/drawings designed for Paw`ina Public Awareness and Fundraising Initiative guests. By participating in this Event, I understand that photography, filming, video, and audio capturing may occur. I, at this moment, grant to Guide Dogs of Hawai`i, its agents, sponsors, partners, and their respective licensees, successors, and assigns (herein collectively called the “licensed parties”) the right to use, publish, and copyright my name, image on video, and film, picture, portrait, voice and likeness in advertising, promoting and publicizing Guide Dogs of Hawai’i in manner or form throughout the world in any form currently in existence or developed in the future, worldwide and in perpetuity. I agree that any picture/video taken of me and they own any recording of my voice by the licensed parties, and I am not owed any compensation. I further agree that no advertisement or other material needs to be submitted to me for any further approval, and the licensed parties shall be without liability to me for any distortion or illusionary effect resulting from the publication of my picture, portrait, voice, or likeness. On behalf of myself, my heirs, and my representatives, I agree to release, indemnify, and hold harmless Guide Dogs of Hawai`i and all of their past and present officers, directors, employees, agents, and assigns from any liability, damage, or claims of any nature that arise out of or are related to my volunteer service. This means that I will not hold the organization or its representatives responsible for any issues that may arise during my volunteer service, to the extent such liability, damage, and claims may be released under the law. By agreeing to this document, I attest that I am 18 or older. This is the legal age for volunteering in our organization, as it ensures that volunteers are of a mature age and can fully understand and fulfill their responsibilities.
