Thank you for volunteering with Paw`ina Hawai`i Public Awareness and Fundraising Initiative hosted by Guide Dogs of Hawai`i (GDH), a non-profit, tax-exempt organization under Section 501(c)(3) of the Internal Revenue Code. This event will take place on Saturday, July 11, 2026, at the Frank F. Fasi Civic Center.
By signing below, you acknowledge and agree to the following:
- Choice of Tasks: You may choose the volunteer tasks that best fit your skills and interests, while also considering the needs of GDH. Your contributions are greatly valued and appreciated.
- Unpaid Service: You understand that you are volunteering without any expectation of wages, compensation, or employee benefits. Your time and effort are given freely and voluntarily to support GDH’s mission.
- Volunteer Status: You acknowledge that you are not an employee of Guide Dogs of Hawai`i and that this volunteer agreement does not create an employment relationship, either now or in the future.
- No Reimbursement: You understand that you will not be reimbursed for any personal expenses you may incur while volunteering, such as parking or meals.
- At-Will Service: Your volunteer service is at-will and may be ended by Guide Dogs of Hawai`i at any time, with or without notice, to ensure a safe and productive environment for all.
- No Provision of Necessities: You understand that Guide Dogs of Hawai`i does not provide food, shelter, clothing, or other necessities of life.
- Background Checks: You may be required to complete a Background Investigation and/or Criminal History Disclosure. Your volunteer service may be contingent on satisfactory results.
- No Participation in Promotions: You understand that volunteers are not permitted to participate in any games, prize drawings, auctions, or similar activities intended for guests.
- Use of Your Image and Voice: You grant permission for Guide Dogs of Hawai`i and its partners to photograph, film, or record you during the event. You agree they may use your name, image, and voice in any promotional or publicity materials, in any format, worldwide and forever, without further approval or compensation.
- Release of Liability: On behalf of yourself, your heirs, and your representatives, you release and agree to hold harmless Guide Dogs of Hawai`i and its officers, directors, employees, agents, and assigns from any liability, damages, or claims arising from your volunteer service, to the fullest extent allowed by law.
- Age Confirmation: By signing this agreement, you confirm that you are at least 18 years old.
- Under 18 Authorization: If the volunteer is under 18 years of age, a parent or legal guardian must give consent. By signing this agreement, you confirm that you are the parent or legal guardian and you give permission for your child to volunteer with Guide Dogs of Hawai`i.
